Lunar mission safety and rescue: Hazards analysis and safety requirements

The results are presented of the hazards analysis which was concerned only with hazards to personnel and not with loss of equipment or property. Hazards characterization includes the definition of a hazard, the hazard levels, and the hazard groups. The analysis methodology is described in detail. The methodology was used to prepare the top level functional flow diagrams, to perform the first level hazards assessment, and to develop a list of conditions and situations requiring individual hazard studies. The 39 individual hazard study results are presented in total.

Manned space flight nuclear system safety. Volume 3: Reactor system preliminary nuclear safety analysis. Part 2A: Accident model document, appendix

The detailed abort sequence trees for the reference zirconium hydride (ZrH) reactor power module that have been generated for each phase of the reference Space Base program mission are presented. The trees are graphical representations of causal sequences. Each tree begins with the phase identification and the dichotomy between success and failure. The success branch shows the mission phase objective as being achieved. The failure branch is subdivided, as conditions require, into various primary initiating abort conditions.

Propulsion safety almost equals mission safety

Propulsion system hardware and monitoring/control software constitute a given manned or unmanned aerospace system's primary risk-management issue. The present inquiry into the reasons for this dominance attempts to identify development routes to the reduction of propulsion-related management risk issues. A 'life management plan' for propulsion systems would give attention to service life requirements, criteria for the monitoring and evaluation of useful life, a method for the tracking of service life, criteria for hardware reusability and operations inspection, and hardware preassembly screening practices.
