Design Considerations for Attitude State Awareness and Prevention of Entry into Unusual Attitudes

Loss of control - inflight (LOC-I) has historically represented the largest category of commercial aviation fatal accidents. A review of the worldwide transport airplane accidents (2001-2010) evinced that loss of attitude or energy state awareness was responsible for a large majority of the LOC-I events. A Commercial Aviation Safety Team (CAST) study of 18 worldwide loss-of-control accidents and incidents determined that flight crew loss of attitude awareness or energy state awareness due to lack of external visual reference cues was a significant causal factor in 17 of the 18 reviewed flights. CAST recommended that "Virtual Day-Visual Meteorological Condition" (Virtual Day-VMC) displays be developed to provide the visual cues necessary to prevent loss-of-control resulting from flight crew spatial disorientation and loss of energy state awareness. Synthetic vision or equivalent systems (SVS) were identified for a design "safety enhancement" (SE-200). Part of this SE involves the conduct of research for developing minimum aviation system performance standards (MASPS) for these flight deck display technologies to aid flight crew attitude and energy state awareness similar to that of a virtual day-VMC-like environment. This paper will describe a novel experimental approach to evaluating a flight crew's ability to maintain attitude awareness and to prevent entry into unusual attitudes across several SVS optical flow design considerations. Flight crews were subjected to compound-event scenarios designed to elicit channelized attention and startle/surprise within the crew. These high-fidelity scenarios, designed from real-world events, enable evaluation of the efficacy of SVS at improving flight crew attitude awareness to reduce the occurrence of LOC-I incidents in commercial flight operations.

A Geofence Violation Prevention Mechanism for Small UAS

The ability to safely confine the trajectories of small UAS to a specific geographical area is a key enabler for capabilities that require operating in close proximity to populated areas as well as other users of the airspace. These capabilities require highly reliable geofencing algorithms. In particular, these algorithms must promptly alert imminent breaches of keep-in/keep-out geofences by considering factors such as the vehicle speed and uncertainties in the state of the aircraft. This paper presents a novel approach to the prevention of geofence boundary violation based on closure rate constraints. These constraints are incorporated into a control framework to effectively prevent fence breaches. Simulation results illustrating an example use case of this framework are presented.

Know Before You Go: A Community-Derived Approach to Planning for and Preventing Sexual Harassment at Oceanographic Field Sites

Sexual harassment is a pervasive problem on oceanographic research vessels and while conducting fieldwork in general. A variety of factors contribute to inadequate protection against sexual harassment, such as poor training in prevention, support, and response; the remoteness of field sites; academic hierarchies that reinforce uneven power dynamics that extend to fieldwork; and multi-institutional teams with distinct policies or reporting structures that can lead to confusion in reporting and responding to incidents in the field. In compromising individuals’ physical and mental health, sexual harassment can negatively affect research expeditions. For example, harassed individuals may decide to refrain from working on complicated team-based tasks, which can be a safety issue. A broader concern is that sexual harassment deters talented people from pursing or maintaining employment in ocean science. Harassment must be treated with the same gravity as research misconduct and safety policy infringements. When planning a research expedition, science team leaders are responsible for the safety of their team and other colleagues aboard and would benefit from resources aimed at helping team leadership create a plan to ensure safety and inclusivity. To address this resource gap and support consistent safety preparedness, 18 participants in the Workshop to Promote Field Safety in Ocean Sciences, convened by the Consortium for Ocean Leadership and held May 17-18, 2022, in Washington, D.C., developed a checklist for use by scientific leaders and others to assist in planning for participant safety and to prevent harassment the field. The checklist specifies the timing of, and who is responsible for, specific actions that should be taken to improve safety while conducting fieldwork, whether on a research vessel or on land. It also provides additional resources and suggestions for leaders on how to amend the checklist to address their specific fieldwork situations.

On an airplane, the safeguards adopted may be divided into three classes, those intended to prevent a fire from getting started, those intended to put it out if it does get under way, and, finally, those designed to limit the damage which a fire can do.

Recommended Values of Meteorological Factors to Be Considered in the Design of Aircraft Ice-Prevention Equipment

Meteorological conditions conducive to aircraft icing are arranged in four classifications: three are associated with cloud structure and the fourth with freezing rain. The range of possible meteorological factors for each classification is discussed and specific values recommended for consideration in the design of ice-prevention equipment for aircraft are selected and tabulated. The values selected are based upon a study of the available observational data and theoretical considerations where observations are lacking. Recommendations for future research in the field are presented.

Environmentally Friendly Corrosion Preventative Compounds

The objective of the Ground Systems Development and Operations Program Environmentally Friendly Corrosion Protective Coatings and Corrosion Preventive Compounds (CPCs) project is to identify, test, and develop qualification criteria for the use of environmentally friendly corrosion protective coatings and CPCs for flight hardware and ground support equipment. This document is the Final Report for Phase I evaluations, which included physical property, corrosion resistance, and NASA spaceport environment compatibility testing and analysis of fifteen CPC types. The CPCs consisted of ten different oily film CPCs and five different wax or grease CPC types. Physical property testing encompassed measuring various properties of the bulk CPCs, while corrosion resistance testing directly measured the ability of each CPC material to protect various metals against corrosion. The NASA spaceport environment compatibility testing included common tests required by NASA-STD-6001, "Flammability, Odor, Offgassing, and Compatibility Requirements and Test Procedures for Materials in Environments that Support Combustion". At the end of Phase I, CPC materials were down-selected for inclusion in the next test phases. This final report includes all data and analysis of results obtained by following the experimental test plan that was developed as part of the project. Highlights of the results are summarized by test criteria type.

Surviving the Improbable: Upset Prevention and Recovery in Flight Control

Slides for an invited presentation (as part of a series) at Santa Clara University, invited by prof. Mohammad Ayoubi. The slides are an overview and summary of past and current research projects in the field of envelope protection, upset prevention and stall recovery guidance, with the aim to avoid loss of control accidents and improve safety in air transportation. The overall aim of this presentation series is to inspire students and to show them possible opportunities that they can pursue for their later careers paths.

NASA Engineering and Safety Center Technical Bulletin No. 15-01: Preventing Incorrect Installation of Polarized Capacitors

Even though NASA’s Lessons Learned Information System has an entry on the reverse installation of polarized capacitors, this error keeps reoccurring and is a “lesson not learned.” The purpose of this bulletin is to discuss some of the reasons this keeps happening as well as offer best practices to prevent this error in the future.

Vented piston seal prevents fluid leakage between two chambers

To prevent fluid leakage around piston seals separating two fluids under differential pressure, a venting system has been devised. Two methods may be used for venting seals through internal passages to an external low-pressure area, O-ring or split-ring seals.

High-pressure regulating system prevents pressure surges

Gas flow is controlled by means of a pressure regulating system which prevents pressure surges. A high-pressure fluid source, a spring-loaded fluid-damped regulator valve, an accumulator, a conventional normally closed command valve, and a control valve are the main components.

Fine-particle filter prevents damage to vacuum pumps

A filter system for mechanical pumps is designed with a baffle assembly that rotates in a circulating oil bath which traps destructive particles. This prevents severe damage to the pump and is serviceable for long periods before it requires cleaning.

Problems of Fire Prevention and Extinguishment in Space Cabins

The problems of prevention and extinguishment of fires in space cabins are, except for a few specific situations, not much different from those at sea level or in aircraft conditions. The unusual atmospheric environment and limitations of space and firefighting equipment compound the general problem. The zero-gravity environment modifies firefighting procedures in a rather profound way and will be given detailed treatment in this report.
