Reviewing clinical considerations and guideline recommendations of C1 inhibitor prophylaxis for hereditary angioedema

Abstract Background Hereditary angioedema (HAE), a rare disease that is characterized by painful and recurring non‐allergic swelling episodes, is caused by the deficiency or dysfunction of C1 inhibitor (C1INH) protein. A comprehensive HAE management plan may require long‐term prophylaxis (LTP) in addition to on‐demand treatment to help “normalize” patients' lives so that they may fully engage in work, school, family, and leisure activities. Aim The main objective of this narrative review is to provide an overview of updated guideline recommendations specific to LTP of HAE and discuss clinical considerations and pharmacologic management options, with a focus on C1INH. Materials and Methods The authors reviewed relevant HAE literature for current recommendations regarding LTP and the role of C1NH. Results Acute HAE attacks are treated with on‐demand medication; however, there is a consensus that LTP should routinely be considered for risk reduction and prevention of future episodes. The 2017 World Allergy Organization/European Academy of Allergy and Clinical Immunology guidelines recommend that all patients with HAE be evaluated for LTP routinely and the 2020 HAE Association (HAEA) guidelines emphasize that the decision to use LTP should not be based on rigid criteria, but rather should be based on individual patient needs. Both guidelines recommend C1INH as first‐line/preferred therapy for LTP in a range of patient types including adults, children/adolescents, and pregnant/lactating patients. The HAEA also recommends the kallikrein inhibitor, lanadelumab, as a first‐line option for LTP. HAE pathway‐specific agents for LTP have not been associated with notable safety concerns. Discussion Plasma‐derived C1INH has been available for 40+ years in Europe and impacts multiple targets within the HAE pathway. C1INH has been used for on‐demand treatment and LTP. A subcutaneous formulation of plasma‐derived C1INH is approved for LTP and produces functional C1INH activity levels consistently above the threshold needed for protection from HAE attacks. Other pathway‐specific options for LTP include the plasma kallikrein inhibitors, lanadelumab‐flyo and berotralstat, approved for adults and pediatric patients aged ≥12 years. C1INH is approved for adults and pediatric patients aged ≥6 years. Conclusion Assessing the need for LTP is vital in the ongoing dialogue between clinicians and patients, as both disease‐related factors and patient preferences may change over time. Among available options for LTP, plasma‐derived C1INH is the broadly recommended first‐line option for LTP in patients with HAE, including pregnant/lactating women and pediatric patients (≥6 years).

Effect of specific non-pharmaceutical intervention policies on SARS-CoV-2 transmission in the counties of the United States

Non-pharmaceutical interventions (NPIs) remain the only widely available tool for controlling the ongoing SARS-CoV-2 pandemic. We estimated weekly values of the effective basic reproductive number (R eff ) using a mechanistic metapopulation model and associated these with county-level characteristics and NPIs in the United States (US). Interventions that included school and leisure activities closure and nursing home visiting bans were all associated with a median R eff below 1 when combined with either stay at home orders (median R eff 0.97, 95% confidence interval (CI) 0.58–1.39) or face masks (median R eff 0.97, 95% CI 0.58–1.39). While direct causal effects of interventions remain unclear, our results suggest that relaxation of some NPIs will need to be counterbalanced by continuation and/or implementation of others.

2022 Kauai Ground Transportation Survey

# 2022 Kauai Ground Transportation Survey The 2022 Kauai Ground Transportation Survey was designed to improve understanding of the transportation preferences of residents and visitors of the Hawaiian island of Kauai. For residents of Kauai, the survey focused on mode-choice decisions for commute, leisure, and shopping activities and whether they would consider using alternative modes instead of driving if such options were available. For visitors, the survey focused on whether they would consider alternatives to rental cars if such options were available. Ultimately, survey results will inform the development of sustainable transportation strategies for the island. ## Data Collection Agency NREL and the Kauai Office of Economic Development conducted the study as part of the U.S. Department of Energy's Energy Transitions Initiative Partnership Project. ## Survey Methodology The survey was conducted July—October 2022. Respondents were recruited via direct email invitation, a press release on the Kauai County website and Facebook page, and posters at community outreach events. ## Survey Records, Data, and Documentation Survey records include 1,437 participants.

Spatial and Temporal Characterization of Activity in Public Space, 2019–2020

The data reported here characterize spatial and temporal variation in the ratio of short-to-long-duration visits in public places (i.e., points of interest) in the United States for each week between January 2019 and December 2020. The underlying data on anonymized and aggregated foot traffic to public places is curated by SafeGraph, a geospatial data provider. In this work, we report the estimated number and duration of “short” (i.e., <4 hours) and “long” (i.e., >4 hours) visits to public places at the US census block group level. Long visits are shown to be a good proxy for workers based on formal economic data. We propose that short visits are more likely to represent nonobligate activities: people visiting a public place for leisure, shopping, entertainment, or civic or cultural engagement. Our work constructs a ratio of short to long visits, which can be used to inform population estimates for nonworker use of public space. These data may be useful for understanding how people’s use of public space has changed during the COVID-19 pandemic and, more generally, for understanding activity patterns in public.

Investigating the influence of latent lifestyles on productive travels: Insights into designing autonomous transit system

As a special case of multitasking, travel-based multitasking typically refers to conducting a set of in-vehicle activities while traveling. Travel-based multitasking has an indisputable influence on offering a pleasant travel experience to transit users during their rides, given that they can use their travel time to perform desirable activities and gain benefits in various form. For instance, the in-activities could help the rider free up time from his/her schedule for the day (i.e., a worthwhile use of travel time). In this study, we investigate how the worthwhileness of a travel-based multitasking could be under the influence of: (1) the transit user’s lifestyle, and (2) socio-demographics, and (3) the characteristics of the transit trip. Towards this, we conducted an intercept survey focusing on the transit trips in the Chicago metropolitan area and analyzed it using latent class modeling approach. Per the results, two classes of transit users could be identified: (1) worthwhileness seekers, productively travelers and (2) leisure seekers, occasional worthwhile travelers. The results also suggest travel time, waiting time and walking distance to the transit station, and the set of in-vehicle activities as significant predictors of worthwhile use of travel time. Finally, the findings provide insights to policymakers for improving public transit systems in the current form, as well as designing an autonomous mobility system as the future form of public transit.
